List the stages of the health promotion process.

1. Reinforce the client's strengths and abilities.

2. Review and summarize information obtained during assessment.

3. Identify health and/or behavioral outcomes indicating the plan has been successful from the client's perspective.

4. Identify health goals and related behavioral change options.

5. Address environmental and interpersonal facilitators and barriers to change.

6. Commit to behavior-change goals, and structure the support needed to meet them.

7. Develop a plan for behavioral change based on the client's preferences, stage of change, and knowledge regarding effective intervention.

8. Reiterate benefits of change, as well as identify incentives for change as the client sees them.

9. Set a time frame for implementation.
