1. Physician's Office Note: Mr. Cafferty presents today with some ascites and complaint of exertional dyspnea. When queried about any other symptoms, he stated that at night he sometimes wakes up coughing, which in his words is described as "real dry." He has a history of congestive heart failure and microvascular spasms. On physical examination, mild ascites, tachycardia, and peripheral edema are noted. Previous diagnostic testing was reviewed, which confirmed cardiomyopathy.
