Skilled Nursing Facility Physician Monthly Progress Note: This patient was admitted in January of last year with a diagnosis of ischemic heart disease, history of bladder cancer, and uncontrolled malignant hypertension. Patient continues to be stable. On exam, he appears comfortable and still has some coughing spells during my physical examination. Vital signs include a weight of 223.8 lbs. Compared to the last 3 months, it varies from 222 to 225. No weight gain or weight loss. I will see the patient in 30 days or at the request of the charge nurse.
