slope (m) = (y₂ - y₁)/(x₂ - x₁) = (0 - 3)/(15 - 0) = -3/15 = -1/5

Point-Slope formula: y - y₁ = m(x - x₁)   ; where (x₁, y₁) is one of the given points

y - 0 = (-1/5)(x - 15)

    -5y = x - 15

             x + 5y = 15

Answer: x + 5y = 15
