1. answered that before: he needs to know if the disorder occurs in the members of family (it would suggest genetic cause of a disease) or in housemates/colleagues from work (it would suggest environmental cause if sick people share the environment)
2. mutation is change in a DNA. ie. deletion of one or several nucleotides or substitution one nucleotide with another. and if substitution causes a situation when the new codon codes the same amino acid as the codon before mutation, then we see no effect of this mutation since peptide looks exactly the same.
