Not sure what the bold portion is, but it looks like the answer is A.
While there is not bolded part of the sentence in your question, it is clear that the answer is that you should use the comma or period inside rule (A).
Why? There is no where in the sentence where a colon, semicolon, question mark, or exclamation point would be appropriate.
In other words, the period should be inside the quotation marks.
"You have a visitor who would like to see you the nurse said."
