I don't have a picture so it's hard to give an exact answer, but you can't really make a triangle or a sphere from a piece of paper unless you have a lot of folds and gluing.
If it's a rectangle or square it would be D. Cylinder. If it's a circle with a slice out if it it would make A. Cone.
