I think the biggest factor would be physical inactivity. Obesity seems like an obvious choice but the fact of the matter is that there are people who are obese who workout daily and have an efficient heart. Physical inactivity is something that could eventually lead up to obesity. Physical inactivity means that you aren't testing out you're heart you aren't giving it a chance to work as efficiently as it can so once a year when someone decides to go running or do something strenuous their heart has to work a whole lot harder to help them do that because its not used to having to pump blood to every part of the body so rapidly. Physical activity is the best way to train your heart and avoid CVD.
