Look at the graph. 

Where x = 12, find what y is.
For example, where x = 10, y is 15.

Now find what y is when x = 15.

You should now have 2 points: (12,y1), and (15,y2).

Now use the slope formula (y2-y1)/(x2-x1) to find the average rate of change. (x1 is 12, and x2 is 15)
