I'm not sure what you mean by 'function table' exactly, but I'm going to give you the formula for functions, and the formule for the average rate of change, otherwise known as the slope of the function.
The widely-used formula for functions is y = mx + b
The formula for finding the average rate of change, the same thing as the slope, is [tex] \frac{y(2) - y(1)}{x(2) - x(1)} [/tex] so you would simply insert the coordinates accordingly.
