In this situation the nurse should encourage the client to calm down and relax also the nurse should promote activities with limited concentration most specially relates to development of delusions.

Some clients who have a problem of mental disorder they have difficulties which self-esteem and trust. The nurse should tell the client words of encouragement to make him or her calm down and be able to relax.
