We know that slope-intercept form is in the format y=mx+b, therefore our new equation must be, too. 
We will let C be our "y," 15(t) be our "mx" and 4 be our "b." Which would look like this:
C=15(t)+4
Which can be more simply written as:
C=15t+4.
